The George Mason University Innovation Inventory Consent Form
You (and your collaborators, if any) are being asked to contribute to the George Mason University Innovation Inventory, which is developing a permanent digital inventory of the projects that have contributed to the tradition of innovation at George Mason University. Your participation in this project will establish a legacy for future faculty, staff, and students at Mason to gain a greater understanding of these events and the people and activities that make the university a world-class institution of higher education.
Contributors must be 18 years of age or older to submit projects to us. The undersigned grants to George Mason University, through its Office of University Relations and Innovation Inventory Committee, the nonexclusive, royalty-free, worldwide right to republish, reformat, distribute, modify, and use in any manner the submitted project materials or any portion thereof, alone or in combination with other material, in any and all forms and media (to include any not yet invented or conceived) for any purpose. Under the following conditions:
- Credit will be given to you (and your collaborators, if any) as contributors of these projects.
- The project materials may be altered, transformed, or built on and the copyright to any such modifications or adaptations will be retained by George Mason University.
- You are hereby granted revocable permission to redistribute and/or republish the George Mason University’s branding or trademarks used in conjunction with these project materials and such must be in accordance with George Mason University’s official visual standards and branding.
You hereby represent 1) that the project materials you submit have been created by you (and your collaborators, if any), are wholly original, and not copied from or based, in whole or in part, on any other photographic, literary, or other material, 2) that you have full power and authority to grant the rights stated above, and 3) that you have not infringed on any copyright, trademark, or other proprietary or privacy right of any other party .
By submitting material to the Innovation Inventory, you release, discharge, and agree to hold harmless the George Mason University, the Innovation Inventory Committee, and University Relations and any persons acting under their permission or authority, including a public library or archive to which the collection might be donated for purposes of long-term preservation, from any claims or liability arising out of the George Mason University’s use of the project materials. The George Mason University Innovation Inventory has no obligation to use your project materials.
You will be sent via e-mail a link to your contribution to the George Mason University Innovation Inventory. The George Mason University Innovation Inventory will not share your e-mail address or any other information with commercial vendors.
